Assessment Package A - Full Assessment £400.00
- Detailed all round assessment (1.5 hours)
- Feedback/Discussion following assessment (1/2 hour)
- Including Fine and Gross Motor Skills, Drawing and Handwriting, Sensory and Perceptual Skills and Activities of Daily Living.
- Detailed Written Report (3 hours)
- Written Recommendations and Programme of Recommended Activities (1 Hour)
- Home/school observation to support assessment available at an additional cost.
Assessment Package B - Full Assessment & Detailed Feedback £550.00
- Detailed all round assessment (1.5 hours)
- Including Fine and Gross Motor Skills, Drawing and Handwriting, Sensory and Perceptual Skills and Activities of Daily Living.
- Feedback/Discussion following assessment (1/2 hour)
- Detailed Written Report (3 hours)
- Feedback Meeting for Parents and/or Nursery/School (2 hours max.)
- Written Recommendations and Programme of Recommended Activities (1 Hour)
- Home/school observation to support assessment available at an additional cost.
Assessment Package C – EHCP Assessment, Review & Report £750.00
- Detailed all round assessment (1.5 hours)
- Including Fine and Gross Motor Skills, Drawing and Handwriting, Sensory and Perceptual Skills and Activities of Daily Living.
- Feedback/Discussion following assessment (1/2 hour)
- Review of existing reports/EHCP (up to 3 hours)
- Detailed Written Report (6 hours)
- Home/school observation to support assessment available at an additional cost.
Assessment Package D - Review Assessment (within 1 year) £350.00
- Detailed all round assessment (1.5 hours)
- Feedback/Discussion following assessment (1/2 hour)
- Including Fine and Gross Motor Skills, Drawing and Handwriting, Sensory and Perceptual Skills and Activities of Daily Living.
- Summary Report (2.5 hours) (Please note this report uses a combination of tables and bullet points.)
- Written Recommendations and Programme of Recommended Activities (1 Hour)
- Home/school observation to support assessment available at an additional cost.
